Having real issues with the headphone outputs on the DSP 24

Seem to operate randomly with no real purpose

Number 1 often seems to be backwards, so will turn up if turned lower and vice versa. Number 2 seems to distort and pan when I turn the knob. Have gone to factory reset just to make sure it's not just some weird settings I've made but seems to have the same consequence regardless of how I change the settings.
Wondering if it's the plugs themselves and the thing has broken?

I'm using a macbook pro retina running El Capitan

Mainly use mainstage and logic but the problem is universal
