It would help if you post details about your system and what version of cakewalk you're using.
If you're using Sonar, go to Options > Project > Metronome Tab.
You can check on playback and/or recording to activate metronome. All versions of cakewalk before sonar 4 sent the metronome to an assiggned midi port. Often, it would be assigned to Microsoft GS Wavetable SW Synth but it can be assigned to any port/synth combination you have.
